
The Kentucky Institute for Patient safety and Quality (KIPSQ) was established in 2008 after the passage of the Patient
Safety Act of 2005. KIPSQ is a federally certified patient safety organization (PSO) through the Agency for Healthcare
Research and Quality.
Our goal is for every provider in Kentucky to join KIPSQ in order to preserve the privilege of conducting self-improvement
and network with other facilities to improve the quality of health care. Kentucky is one of two states with no Peer Review
protections — KIPSQ provides a legally protected environment for healthcare facilities and providers to candidly discuss
why adverse events occur and how they can be prevented.
One of the first PSOs in the nation, KIPSQ is leading patient safety efforts in Kentucky through analysis, dissemination
and archiving of patient safety information. KIPSQ offers access to the emerging best practices of hundreds of hospitals
in the USA.
K-HEN is one program under KIPSQ, created by CMS in conjunction with American Hospital Association Hospital
Research and Education Trust (HRET) to synchronize quality improvement and patient safety efforts. Learn more

Another exciting program is the Patient Safety for Mom and Baby. KIPSQ has partnered with the Anthem Foundation
to provide education, tools and resources to improve care and long-term outcomes for mothers and newborns. The
Anthem Foundation has provided grant funding for hospitals to engage in a collaborative patient safety and quality
program aimed at reducing early elective deliveries (EEDs), reducing obstetrical harm by preventing unnecessary
cesareans, reducing adverse outcomes from oxytocin use and reducing OB hemorrhage. Additionally, resources
will be provided to hospitals to improve the rate of breast feeding.
Complimentary membership is available to Kentucky Hospital Association (KHA) Critical Access Hospital members
and Kentucky Hospital Insurance Company (KHIC) members. All other memberships are available for a fee. Contact

receive better and safer care with improved outcomes, thereby resulting in healthier individuals and communities.
Mission
The Kentucky Institute for Patient safety and Quality is dedicated to quality and patient safety activities to improve the
way health care is delivered. This will be accomplished through the conduction of activities which include the collection
and analysis of data to identify problem areas and opportunities for process improvement: the development and
dissemination of best practices and reliable information to improve safety and quality; and to foster a culture of safety and
a learning environment for persons engaged in the delivery of health care services.
